I am on a Fiance Visa and ready to submit form I-485. Do I need my original documents?

I have read over the evidence required for form I-485 Adjustment of Status and form I-765 Employment Authorisation and included in the Evidence Required is my birth certificate and criminal record. However, these were taken from me upon entry into the United States on my Fiance Visa. What do I do in this situation? And also, what is the Vaccination Supplement (My guess would be a doctors note stating that I've had the required jabs before I came here, but clarification would be great)?
